Documents Required:
1) Change in Address:
  • Your Original National Identity Card (CNIC)
  • Original CNIC of your Blood-Relative or Wife or Husband, with same address
    OR… Original Utility Bill on your Name & Permanent Address
2) Change in Date of Birth:
  • Your Original National Identity Card (CNIC)
  • Child Registration Certificate (Form-B)
  • Your Matric Certificate or Matric Marks-Sheet or Medical Certificate or Passport

Please Note:

  • Date of Birth cannot be changed if age is 45 and above
  • You can change your Date of Birth only ONE time
  • If you have unnatural age with real brothers & sisters, you can change your Date of Birth upto one year (365 days)
  • For Date of Birth, no document is required for change in Day and Month
3) Change in Marital Status:
  • Your Original National Identity Card (CNIC)
  • In case of your marriage, Nikahnama or Marriage Certificate or Affidavit or Husband’s CNIC
    (If Nikahnama or Marriage Certificate is not available then bring wife or husband with you)
    (If wife or husband is not available then bring with you any one from in-laws / parents with CNIC)
    OR
  • If divorced, Certificate of Arbitrary Council or Affidavit
    OR
  • If Husband died, Death Certificate of Husband/ Affidavit
4) Change in the Name:
  • Your Original National Identity Card (NIC)
  • Child Registration Certificate (Form-B)
  • Matric Certificate or Matric Marks-Sheet or Medical Certificate or Passport
  • Affidavit attested by Oath Commissioner / Certificate from Daarul-Afta (due to religion change only)

Please Note:

  • Bring with you at NADRA office any blood-relative as father/mother/brother/sister/son/daughter, with their CNIC
  • Operator will enter your data and give you its Print
  • The blood-relative will attest your above Form/ Print
  • Male applicant can change his name only ONE time
  • Female applicant can change her name many times (due to change from father to husband name)
5) Change in Gender (Male/Female):
  • Depends upon desire or appearance of Applicant
6) Change in Religion:
  • Your Original National Identity Card (CNIC)
  • Affidavit/Certificate from Darul-Afta (converted to Islam)
    OR
  • System Generated Undertaking or witness of same religion (from Islam to other religion)
7) Change in Parent’s Name / Information:
  • Your Original National Identity Card (CNIC)
  • Child Registration Certificate (Form-B)
  • Matric Certificate or Matric Marks-Sheet or Medical Certificate or Passport
  • Original CNIC of parent. Also bring the parent with you at NADRA office with their CNIC

Please Note:

  • You can correct the names of parents (alive or not alive)
8) Change in your Signature on your CNIC:
  • Your Original National Identity Card (CNIC)
  • Two photocopies of CNIC
Steps:
  1. Pay the Fee and get Token Number from counter
  2. Staff will take your fresh photograph
  3. Officer will take your fresh fingerprints & signature
  4. You will surrender / submit your original CNIC
  5. NADRA sends SMS on your mobile that CNIC is ready
  6. Receive your CNIC personally from NADRA Office on date
Tips:
  • For fingerprints, avoid mehendi / dust / oil on your fingers
  • Keep with you enough change of money
  • Keep with you a pen

    sir ma ap sa apna id card ka bara ma kuch information chahti hn. mara id card pr mara single name tha kuonka mara matriculation certificate or B FORM pr start sa single name tha . but in BSC mari teacher na mara single name pr aitraz krta hoa mara father name ka last name mara name ka sath form pr likh kr bhijwa dia ijs ki wja sa mara matirc or bsc ka result pr name change ho gia, phir jb mujha further masters ka lia apply krna para to name double hi rha then job ma bhi tamam documents pr double name rha . ab jab ma na shadi ka bad apna husband name ka sath apna double name ma change krwaya ha to kia yun 3 different name sa koi problem to nhi ho gaa. kindly guide me

      W.Salam … Pakistan main rehtay huay tau shaayad aysa koi problem na ho, lekin country say bahir jaatay huay visa waghera main ya foreign country main job kay waqt problem ho bhi sakti hay. Lekin amooman woh log apni satisfaction kay liay koi izaafi documents bhi maang letay hain aur aap ka problem solve ho jata hay.

      Aaj kal dunya bhar main yeh rujhaan (trend) hay keh aap ka naam two ya three hisson (parts) pay mabni ho. Isi liay shaayad B.Sc. main teacher nay ya Board/University nay demand kia ho keh students kay naam minimum do (two) hisson pay mushtamil hon.

      Yeh ziada behtar hoga keh aap dobara CNIC pay apna naam single yaani Sabeen hi karwa lain. Agar aap future main mazeed koi degree hasil karti hain tau phir iss new degree pay aap ka naam father kay naam kay saath aega aur B.Sc. aur Masters degrees say clash nahi hoga. Matric aur Intermediate pay agar single name hay tau koi baat nahi kiyonkeh Matric aur Inter main father name bhi mojood hay.

      Hamaari tajaveez final nahi hain. Aap apnay friends waghera say mazeed discuss kar sakti hain taakeh proper solution tak pohonch sakain.

      Aap apnay father kay saath qareebi NADRA Centre tashreef lay jaen aur yeh documents bhi saath lay jaen:-

      1) Adopted Bhai ka original CNIC
      2) Aap kay father ka original CNIC
      3) Agar ho sakay tau Chacha ka original CNIC ya photocopy ya CNIC No.
      4) Agar mojood ho tau Bhai ka B-Form
      5) Bhai ka Union Council ka jaari karda Birth Certificate

      Agar mazeed kisi document (maslan Affidavit ya Halaf-nama) waghera ki zaroorat hogi tau NADRA waalay aap ko mukammal guide kar daingay aur aap ka kaam aasaan hojaega, InshaAllah.

            W.Salam … Form jama hogaya. It is good. Agar koi objection ya etiraaz hoga tau NADRA walay aap ko bata daingay. Mumkin hay koi bhi objection na ho. Aap ko CNIC 30 days main mil jaega. Agar CNIC na milay tau 30 days kay baad NADRA Centre jaen, InshaAllah mil jaega.

          5. السلام عليكم ۔ سر ! میں اپنی شناختی کارڈ میں موجودہ ایڈریس تبدیل کرنا چاہتا ہوں ۔ میرا موجودہ ایڈریس کراچی کا ہے۔ جبکہ میں پشاور میں اس وقت جاب کرتا ہوں ۔ تو کیا طریقہ کار ہوگا کہ میں اپنا موجودہ ایڈریس پشاور کا بنادوں؟ جبکہ میں یہاں پشاور میں تقریباً گزشتہ دس سالوں سے ایک ادارے میں جاب کرتا ہوں ۔
            تو کیا اس ادارے کی طرف سے یہ تصدیق کافی ہوگی کہ یہ ہمارے یہاں اتنی مدت سے جاب کرتا ہے یہاں کا رہائشی ہے؟
            یا اور کوئی چیزیں بھی ضروری ہیں؟ کیا طریقہ کار ہے اب میرے لئے موجودہ ایڈریس تبدیل کرنے کے لئے؟

            Agar aap ka makaan rent pay hay :-
            1) Aap ka original CNIC

            3) Makaan Maalik kay naam pay original Utility Bill aur photocopy
            4) Makaan Maalik kay CNIC ki photocopy
            5) NADRA koi Halaf-nama maangega jis pay makaan maalik aur aap kay sign hon

            Agar makaan aap ki family ka hay :-
            1) Jis family member kay naam pay makaan hay iss ka original CNIC
            2) Original property documents (NADRA sirf 3-pages ki photocopy lega)
            3) Property Maalik kay naam pay original Utility Bill aur photocopy
            4) Property Maalik kay CNIC ki photocopy
            5) Jab ek family member ka address change hojae tau phir sab ka karwa saktay hain

            Agar NADRA aap say demand karay keh aap kay Office ki tasdeeq bhi karwain tau aap aasaani say karwa saktay hain.

            Aap yeh tamaam documents lay kar NADRA Centre tashreef lay jaen. Woh log aap ko mukammal guide karaingay aur aap ka address Peshawar ka hojaega, InshaAllah.

    My all educational documents from matric to BS has my fathers name as “Khalid Akram” but my Cnic has “Rana Khalid Akram”. Now I want to change it from Rana Khalid Akram to Khalid Akram. It is because i want to pursue my masters from abroad. Will Nadra change the name of my Father.?

    1. Dear Mr.Inzamam Khalid,

      W.Salam … If your father’s name is written on his CNIC as “Rana Khalid Akram” then, perhaps, firstly your father will have to change his name from “Rana Khalid Akram” to “Khalid Akram” on his CNIC. Then you will be able to get changed your father’s name on your CNIC.

      Therefore, you are suggested to contact personally the nearest NADRA Centre. They have their own rules & ways and it is quite possible that they will change your father’s name as “Khalid Akram” on your CNIC. My be, they don’t insist you to change your father’s name on his CNIC. Please contact NADRA and we hope they will try to resolve your problem at their level best, InshaAllah.

  8. hello sir basically i belong to bhakkar and now i live in lahore in rental house and i have rent agreement so its possible to change adrees in my id card presesnt adress is lahore.

    1. Dear Mr.Ghulam Sabir,

      You can change your present address of Lahore and your permanent address will remain the Bhakkar City. Please visit nearest NADRA Centre in Lahore with following documents:-

      1) Your original CNIC
      2) Original Rent Agreement and photocopy
      3) Original Utility Bill in owner’s name and photocopy of Bill
      4) Photocopy of property owner’s CNIC
      5) NADRA can take an Affidavit having yours and owner’s signatures

      You will receive your fresh CNIC within 30 days and the fees for CNIC is Rs.400/- only.

    My problem is that my father passed away,His Name: Qalab Ali Asad,I want to Chang the only third name ,Asad on my ID card,because only Qalab Ali is written on my all educational documents,not only,me,my sis,brothers are educated with same condition,
    Q.is how I can remove my father third name “Asad”from Nadra please guide please.

    1. Dear Sir,

      If you will contact NADRA, they will advise you properly. However, we can also discuss here about removal of your father’s last name “Asad” from your CNIC.

      On your CNIC, your father’s name is written “Qalab Ali Asad” but on your educational documents it is written “Qalab Ali” only. Normally, if you are in Pakistan, this difference of name does not create any problem. But, at any stage, if you present your documents to any embassy for travelling abroad for a visit or employment, this difference may possibly create some problem. The embassies or other institutions may ask you for further supporting documents. It consumes some extra time and efforts, but people are normally able to arrange these additional supporting documents.

      Keeping in view the above arguments, we hope you will be able to plan in a better way. If you decide finally, you can apply NADRA for removal of your father’s last name “Asad” from your CNIC. As your father is not alive, NADRA may ask you to present some extra documents or any kind of Affidavit. But it is quite possible that they will amend your CNIC as per your desire.
